#1  Go to the following link, select Athletic Team Registration and fill out all required fields.

#2  Get a doctors physical and upload the physical form to the website.

#4  Impact Tests are required one time during your high school career.

• Go to the website impacttestonline.com/testing
• Enter the CCPS customer code of TJE7DPTVNZ
• In the drop down menu for SELECT YOUR ORGANIZATION, select “WESTLAKE”
• Complete the test (approximately 30-40 minutes)
• Please send an email to apeck@ccboe.com to let me know that you have completed the test.
